This is the "forest" view of the medical record, to give the physician a longitudinal perspective of a patient's health.

The Spreadsheet format allows the physician to quickly assess disease progression, patterns of illness and/or therapeutic efficacy.

Each visit is one row in the Spreadsheet.

The columns present the key information for a visit. The column "headings" are based on "characteristics" related to the primary diagnosis of the patient AND designed for the specific specialty and practice.

Custom columns specific for a particular patient may be appended to the standard columns.

• Key information for a visit is entered directly into the table.
  
• Certain calculations are performed automatically, e.g. height SDS, velocity and BMI.
  
• Images, reports of diagnostic studies and lab results can be associated with the visit, and viewed while reviewing the medical record
  
• The Visit can be "signed" electronically.
