When considering the costs of providing medical services, medical liability and malpractice insurance loom in the forefront.  Because of the rate of litigation and the growing size of settlements and damages, these expenses are rising more rapidly than reimbursement in almost every state.  This has caused many doctors to leave practice altogether because they can no longer obtain insurance affordably, if at all. 

Most cases of liability derive from errors of (1) omission, e.g. failing to document an allergy that later results in serious reaction, or failing to provide preventative care, or (2) commission such as not recognizing a potential drug-drug or drug-diagnosis interaction.

Thus documentation is at the root of many of these cases: most are lost by the physician as a result of poor record-keeping.  Improved documentation alone has the potential to annually save billions of dollars and, more importantly, thousands of lives.
